Church Equipment Loan Form

PROCEDURE:
1. A Church Equipment Loan Form must be filled out and submitted to the church office at least two weeks in advance.

2. Upon approval, someone from the church office will contact you, arranging a time during office hours (8:00 a.m.-4:30 p.m. Monday-Thursday, 8:00 a.m.-noon Friday) when the requested equipment may be picked up.

3. The equipment must be returned to the church during office hours no later than 24 hours after the event or by agreed time.

4. Any damages to the church equipment will be the responsibility of the borrower.

5. Certain pieces of equipment will have a fee charged for usage.
